During the third phase of the project (2018), a formal inter-institutional network was established in Doboj municipality that can identify patients in need and promote available services through dialogue with the patients and their families in order to develop a common care plan. This plan includes a "quality of life contract" to ensure the provision of services in accordance with the needs of patients, their preferences and expectations. In order to achieve this goal, it was necessary to do the following: (i) create a database of patients and services available, (ii) develop a joint care plan with quality criteria, and (iii) educate, train and supervise medical workers involved and other actors.

Management of the Health Center Doboj has appointed five nurses in the city of Doboj and 8 nurses in the villages of the municipality for the work with terminally ill patients. In order to develop knowledge and skills in palliative care, the experts from the University Hospital of Geneva (HUG) organized training for doctors and nurses of the Health Center Doboj based on the European palliative care plan and based on the expressed needs of the medical staff. The staff of the Health Center Doboj, with the help of palliative care specialists from the Geneva Hospital, produced a brochure with the 9 most common symptoms in patients in the terminal phase of their lives. The involved health workers were supported in their work through regular supervision and regular team meetings.

In February 2018 Orasje Health Center has expressed willingness to join the project. The Health Center Orasje is not able to provide all necessary aspects of care and satisfy the basic needs in medicine, help, treatment of pain, social and other problems of terminally ill patients. The Foundation fami and HUG decided to include the Orasje Health Center in the project and to assist them in the process of improving palliative patient care.

We started to research the expectations of patients, their needs and existing end-of-life care services and mapped all stakeholders involved in response to the needs of patients in the municipality of Orasje (health workers, Ministry of Health, Labor and Social Welfare, the Municipality of Orasje, Social Welfare Centre, the Health Insurance Institute of the Posavina Canton, the Red Cross, Merhamet, religious communities, etc.).
