The Form was co-created by Fondacija fami, EOLC teams in Primary Health Care Centres of Orasje and Doboj, specialists in palliative care and staff of the Division of tropical and humanitarian medicine of the HUG. This form can be downloaded and used for the needs of terminally ill patients.
